Serine-threonine/tyrosine kinases, transforming growth factor ß1 signaling pathway, lipid rafts in the plasma membrane, exosomes released from tumor cells, and tumor hypoxia are exciting targets in cancer therapy. Following the recognition of protein kinase C (PKC) as the receptor for the tumor-promoting phorbol esters and the identification of the physiologically relevant substrates of its isozymes, we now have a better understanding of the mechanism of action of this family of serine-threonine kinases. PKC kinases regulate a variety of cell functions including proliferation, gene expression, cell cycle, differentiation, cytoskeletal organization, cell migration, and apoptosis. Several PKC isozymes including a, bII, e, i?, and h appear to be pro-oncogenic in skin, colon, prostate, ovarian, breast, lung, and brain cancers, while PKCd appears to have some tumor inhibitory characteristics in most cancers except that of the brain. The pro-oncogenic function of these isozymes in different malignancies is addressed. The complex role of the stroma in mammary carcinogenesis is discussed. These observations challenge the somatic mutation theory of carcinogenesis, suggesting that cancer is caused by an initiatory genetic event in a single cell, followed by a series of additional genetic events, culminating in clonal expansion of the initiated cell. Recently, it has been suggested that carcinogenesis occurs at the tissue level of biological organization, implicating both the epithelial and stromal compartments. Furthermore, the stroma is no longer regarded as an innocent bystander, but an active participant in the tumorigenesis process, providing a hospitable environment for the initiated cells to progress and express their neoplastic potential. Radiation or other stromal remodeling factors may contribute to neoplastic progression in vivo. The key role of sphingomyelin, a main component of the plasma membrane, in the generation of ceramide upon exposure to apoptotic agents is described. Ceramide, in turn, acts to facilitate death receptor clustering and downstream cell death signaling. Sphingomyelin also plays a role in determining the composition and involvement of lipid rafts in controlling the uptake of alkylphospholipids, a new class of apoptosis-inducing synthetic drugs. It has been known for decades that the immune system elicits antitumor responses, but more recently it has been found that these responses are compromised in patients with cancer due to abnormalities in their immune systems. These abnormalities, which are evident in patients with advanced cancers, may contribute to tumor escape from the immune system s surveillance and elimination. Evidence is presented as to how tumor-derived exosomes or microvesicles contribute to immune dysfunctions and thus may be considered as one of the mechanisms responsible for tumor escape from the host immune system. With acute radiation syndrome (ARS) as the focus, the status of pharmacological countermeasures that use novel radioprotectants, radiation mitigators and post-radiation treatments is presented within the framework of the existing knowledge of ARS biology. The impact of radiation-induced bystander effects on estimation of radiation cancer risk and outcomes of radiation therapy is described. Her expertise in drug development spans a wide range of approaches including immunotherapies, gene and protein therapies, and small molecule delivery with publications in journals such as Molecular Cancer Therapeutics, Cancer Research, and Microvascular Research . Her current research focuses on stem cells, tumor vasculature, and target validation. Dr. Beverly A. Teicher is Vice President of Oncology Research at Genzyme Corporation. Dr. Teicher completed a PhD in Bioorganic Chemistry at the Johns Hopkins University and postdoctoral training at Yale University School of Medicine. Dr. Teicher joined Dana-Farber Cancer Institute as an Assistant Professor of Pathology and rose to Associate Professor of Medicine and Radiation Therapy, Harvard Medical School at Dana-Farber Cancer Institute and Joint Center for Radiation Therapy. Dr. Teicher is an active member of the international scientific community having authored or co-authored more than 400 scientific publications. She has edited eight books, is senior editor for the journal Clinical Cancer Research and is series editor for the Cancer Drug Discovery and Development book series.

Book excerpt: A critical review is attempted to assess the status of nanomedicine entry onto the market. The emergence of new potential therapeutic entities such as DNA and RNA fragments requires that these new “drugs” will need to be delivered in a cell-and organelle-specific manner. Although efforts have been made over the last 50 years or so to develop such delivery technology, no effective and above all clinically approved protocol for cell-specific drug delivery in humans exists as yet. Various particles, macromolecules, liposomes and most recently “nanomaterials” have been said to “show promise” but none of these promises have so far been “reduced” to human clinical practice. The focus of this volume is on cancer indication since the majority of published research relates to this application; within that, we focus on solid tumors (solid malignancies). Our aim is critically to evaluate whether nanomaterials, both non-targeted and targeted to specific cells, could be of therapeutic benefit in clinical practice. The emphasis of this volume will be on pharmacokinetics (PK) and pharmacodynamics (PD) in animal and human studies. Apart from the case of exquisitely specific antibody-based drugs, the development of target-specific drug–carrier delivery systems has not yet been broadly successful at the clinical level. It can be argued that drugs generated using the conventional means of drug development (i.e., relying on facile biodistribution and activity after (preferably) oral administration) are not suitable for a target-specific delivery and would not benefit from such delivery even when a seemingly perfect delivery system is available. Therefore, successful development of site-selective drug delivery systems will need to include not only the development of suitable carriers, but also the development of drug entities that meet the required PK/PD profile.
