Antifolate drugs that are largely transported via a high capacity folate transporter (i.e. the reduced-folate carrier) and inhibit folate-dependent enzymes include the dihydrofolate reductase inhibitor, methotrexate, and the thymidylate synthase inhibitors, raltitrexed and pemetrexed. Major advances in folate research made within the last decade include (i) the approval of pemetrexed for the treatment of lung cancer and mesothelioma, and (ii) the demonstration that cell membrane-anchored folate receptors (FR) are exploitable for cancer and inflammatory disease management. FRs are not widely distributed in normal tissues, except on some luminal surfaces; however, they are accessible to systemically administered agents when expressed on many cancers as well as on activated macrophages involved in various inflammatory diseases. High affinity folate-radioisotope conjugates have been developed for imaging pathogenic FR-positive diseases, including cancer. Since the FR transports folates via a low capacity but high affinity endocytic pathway, a variety of FR-targeted antifolate drugs and folate conjugates bearing a wide range of payloads (including cytotoxic drugs) are currently being developed which exploit this property. The FR is also being utilized in immunotherapy approaches for the treatment of overexpressing cancers.

Book excerpt: In recent years, nanoparticle-based drug delivery systems have gained remarkable attention for cancer treatment since they can improve the safety of highly toxic anti-cancer agents. Conventionally, nanoparticles delivery of anticancer drugs to tumor tissues can be achieved by the enhanced permeability and retention effect. However, this passive mode of drug delivery has intrinsic limitations, such as tumor heterogeneity, that diminish tumor targeting specificity. A new concept has emerged, that of nanotherapeutics which selectively target immune cells to mediate drug delivery into tumors. This work focuses on a novel strategy to increase the delivery of nanotherapeutics to tumors by using neutrophils loaded with chemotherapeutics-containing nanoparticles to target photoactivatable tumors, and by taking advantage of the acidic microenvironment of tumors for selective pH-dependent drug release. The novel strategy we studied combines photodynamic therapy to stimulate inflammation around tumors and attract neutrophils charged with acid-pH sensitive, drug-containing, protein-based nanoparticles. Photodynamic therapy usually consists of three components: a photosensitizer, light and tissue oxygen. We proposed a novel approach by combining monoclonal antibody-photosensitizer conjugates and the laser light to induce inflammation in a tumor. For the targeted release of chemotherapeutic drugs, we designed a novel model of drug-containing nanoparticles in which the release of the drug is selectively activated by the acid microenvironment of tumors. This model was studied and validated using DOX-cis-BSA nanoparticles as prototypes. In DOX-cis-BSA nanoparticles, doxorubicin (DOX), a widely used chemotherapeutic agent, is attached to bovine serum albumin (BSA) via a pH-sensitive linker, cis-aconitic anhydride (cis). This linker readily hydrolyzes in the acidic tumoral environment, thus providing tumor-selective release of DOX. In summary, this thesis is aimed at developing a new type of nanoparticles that can be internalized by neutrophils to mediate the delivery of an anticancer drug to tumors after photodynamic therapy. Our work demonstrates that 1) inflammation can be successfully induced by laser-light in tumors labeled with tumor-specific monoclonal antibody-photosensitizer conjugates, and 2) acid-pH sensitive DOX-cis-BSA nanoparticles can be successfully synthesized. Our data provide feasibility evidence for the development of a novel delivery platform for chemotherapeutic drugs and suggest potential effectiveness of our new approach in cancer treatment.

Indeed, acute self-resolving inflammation usually plays a positive role for the host, as exemplified by infectious diseases where its positive role is well established and testified by its perception as innate immunity. On the other hand, non-resolving inflammation and consequent chronicization is a key determinant of immunopathology and clinical manifestations of most major diseases in humans. As a consequence, it is increasing appreciated that the problem with inflammation is not how often it starts, but how often it fails to resolve. Appropriate resolution of inflammatory responses, which also drives activation of tissue damage repair mechanisms and return of local tissues to homeostasis, is a necessary process for ongoing health. Interestingly, cells sustaining these processes are also key to the proinflammatory responses, and the underlying “pro-resolving” molecular pathways are triggered as part of the pro-inflammatory response. This clearly indicates resolution of inflammation as an active process requiring functional repolarization of inflammatory cells that calls our attention on the underlying molecular mechanisms. The increasing number of anti-inflammatory drugs best-sellers in the pharma market is a clear indication of the relevance of having inflammation under check; nonetheless, there is still a great need for better acting pharmacological tools for the control of inflammation. Indeed, the remarkable success of biological drugs targeting proinflammatory cytokines has indicates that tools able to block proinflammatory mediators have promising applications, but at the same time has made clear that there are intrinsic limitations to this approach which frequently vanish undermine the activity of single targeting drugs, including the well-known redundancy of inflammatory mediators. Under self-limiting conditions inflammation spontaneously resolves in an active process. Some cellular and molecular mechanisms involved in inflammation resolution have been uncovered in the recent past, and include generation of specific cytokines, apoptosis of inflammatory leukocytes, lipid mediators, macrophage repolarization and others are likely to be revealed in the next future, since loss-of-function mutations of an increasing number of genes results in the development of spontaneous inflammation in experimental animals. We argue that “pushing for“ inflammation resolution by exploiting active naturally-occurring pro-resolving processes may have significant advantages over the attempt to simply “push back” inflammation by passive blockade of proinflammatory mediators. At present the research in the field of inflammation aims at identifying and validates new molecules involved in the resolution of inflammation as a basis for the development of innovative therapeutic strategies in chronic inflammatory and autoimmune diseases. This involves the discovery of new natural or synthetic “pro-resolving” molecules from plant and animals and the investigation of endogenous inflammation “pro-resolving” mechanisms, including atypical chemokine receptors, decoy receptors, and microRNA.
